commands response: "There was a problem with the app."
Fix 1: Update the
apps
The siri response "There
was a problem with the app." can occur when the app in your iPhone is
outdated. You must check in Appstore about latest version of the apps. Some of
the apps keep updating themselves to support Siri. Some users have reported
that V2.1.1 of shortcut app has fixed many Siri related issue.

reinstall the app
If your iPhone have gone
through a recent iOS 12 update it is possible that there might have occurred some
software issue while registering the apps during upgrade. So it is highly recommended
to re install the affected apps which are not responding to Siri commands.
Fix 3: Create a
duplicate shortcut
As a workaround, you
should create a second shortcut commands to serve as the Siri activation. This
can help you to do same thing without any issue.
Fix 4: Try using
“continue shortcut in app” at the start of anything non-trivial
Edit the shortcut. At
the bottom type “continue shortcut”. Add continue shortcut in app to your
shortcut and drag it to the top.
Fix 5: Update iOS to
latest version

raised with Apple and apple support have confirmed that the issue is widespread
and Apple engineering is looking at it. So possibly Apple can roll out a fix in
upcoming releases that you should not miss.
